IT IS THE CLIENTS RESPONSIBILITY TO READ AND UNDERSTAND ALL INFORMATION PROVIDED CAREFULLY PRIOR TO BOOKING ANY SERVICE.

YOUR HEALTH AND SAFETY IS IMPORTANT AND PRIORITY. ANYONE WHO ARE OR HAVE THE FOLLOWING LISTED BELOW WILL NOT BE ELIGIBLE FOR ANY PERMANENT MAKEUP SERVICES. BOOKING AN APPOINTMENT WITH ANY OF THESE CONDITIONS WILL RESULT IN CANCELLATION AND DEPOSIT WILL BE FORFEITED.

• Under 18yrs of age
• Pregnant, nursing, or planning to conceive
• COVID-19 Virus/symtoms
• Allergic reactions to Tattoo Pigments, Topical Anesthetics, or Makeup
• Allergic reactions to Lidocaine or Epinephrine
• Allergic reactions to metal and/or color
• On any blood thinning medications, or long term use of aspirin, Tylenol, and/or ibuprofen
• Botox/Chemical Peels/Facials/Lasers/Microdermabrasions within 1 month of service
• Any Skin conditions around treatment area such Eczema, Dermatitis, Rosacea, psoriasis, etc)
• Any Open Wounds, rashes around the treatment area
• Use of Accutane/Retin A within the last year
• Blood Disorders
• Healing Disorders
• Diabetic (insulin dependent)
• Epilepsy (prone to fainting)
• Glaucoma
• Heart Conditions
• HIV, HEPATITIS or any transmittable blood diseases
• Hyper Pigmentation or Hypo Pigmentation
• Post inflammatory Hyper Pigmentation
• Immune and Healing Disorders
• Lupus
• Prone to Keloids
• Active Skin Cancer, Undergoing Chemo, Radiati
• Undergoing Organ Transplants
• Any other viral infections or diseases

For any other medical conditions you feel might be a contraindication, please consult with your Physician before considering any permanent makeup services. If approved, be sure to provide a Doctor's Approval Note(required) at the time of your Service/Appointment or we are unable to perform any service, your deposit will be forfeited and result in cancellation.

COVID-19 VACCINATION

If you have received or plan on taking the COVID-19 VACCINATION, please allow yourself to wait 14 days before/after any permanent makeup service. 

FEELING SICK?

Please stay home and reschedule. If you are feeling sick and/or showing any symptoms at your appointment, you will be sent home, appointment will be cancelled, and your deposit will be forfeited.

• DEPOSITS:
• (Paramedical Tattoo Deposit)  $185 NON-REFUNDABLE AND NON-TRANSFERABLE DEPOSIT is REQUIRED to secure your appointment and will go towards your service cost. 
• (Brow Deposit)   $100 NON-REFUNDABLE AND NON-TRANSFERABLE DEPOSIT is REQUIRED to secure your appointment and will go towards your service cost.
  
• Remaining balance is to be paid in FULL on the day of appointment. Forms of payment: CASH/ZELLE or Credit Card with a 3.75% transaction fee.
• NO GUEST, NO PETS are ALLOWED to your appointment. We want the best results for you and your brows. Having additional guest/pets will cause distractions during the procedure. 
• EXISTING BROW TATTOO/MICROBLADING DONE BY ANOTHER ARTIST, PICTURES ARE REQUIRED FOR APPROVAL. BOOKING ANY SERVICE WITHOUT ANY PICTURES SENT WILL END UP LOSING DEPOSIT AND/OR CANCELLATION.
• ALL DEPOSITS ARE NON-REFUNDABLE AND NON-TRANSFERABLE
• ALL SERVICES ARE NON-REFUNDABLE AND FINAL
• RATES/PRICING ARE SUBJECT TO CHANGE AT ANYTIME WITHOUT NOTICE AND ARE NON-NEGOTIABLE. 
• PLEASE BRING YOUR ALOHA, I HAVE THE RIGHT TO REFUSE SERVICE TO ANYONE. 

I UNDERSTAND THINGS MAY COME UP AND UNFORESEEN EVENTS HAPPEN. PLEASE PLAN AHEAD WHEN BOOKING YOUR APPOINTMENT. PLAN ENOUGH TIME TO BE AT LEAST 10-15MINS EARLY TO YOUR APPOINTMENT.

• LATE: Please give me a Courtesy CALL/TEXT if you are running late. If you are running 15-20mins late, please be advise that there will be a $25 LATE FEE additional to your remaining balance. Anytime more than 25mins late, your appointment will be cancelled, your deposit will be forfeited, and you will be required to pay an $85 rescheduling fee. We value our time and your time as much as the next client, please be on time. No Refunds, No exceptions. So please be on time and plan ahead.

• RESCHEDULING: If you need to reschedule your appointment, please inform me no later than 48hrs of your appointment. If it is less than 48hrs, there is an $85 Rescheduling Fee. You may reschedule once to keep your deposit towards your service. The second time rescheduling will require a new deposit of $100 and it will NOT go towards your service. Keep in mind when rescheduling, service pricing may change without notice, old service price will not apply unless a deposit was made before the change..

• CANCELLATION:  If you need to cancel your appointment, please inform me no later than 72hrs of your appointment. Canceling less then 48hrs, your deposit will be forfeited and you will need to re-book online requiring a new $100 deposit. No Refunds, No exceptions. 
• SAME DAY CANCELLATION: There is a charge of a $150 Cancellation Fee.

• NO CALL/NO SHOW: Your deposit will be Forfeited. You will be charged half the price(50%) of your service appointment. No future appointments.
